Prepare the Family Card that includes the child’s name

Please upload a clear and readable Family Card. The child’s name must appear on the Family Card so the secretariat can verify the child dedication data more quickly.

The data and document you submit are only used for ministry verification and administrative purposes.

Once submitted, the data will be saved to the database and emailed automatically to the church secretariat.

Make sure the Family Card document is not blurry and the child’s name can be read clearly.
